Kit- Rotate by Gimptastic Scraps. You can find this edgy kit at The PSP Project here. Please respect the designer and do not share this PTU kit.
Tube- Rotate by KiwiFireStorm. This tube can be purchased at The PSP Project here. You need a valid license to use his artwork.
Template- Tag Template 238 by Toxic Desirez here
Mask- Mask of choice ( I think I used one by WSL but can't remember)
Font- Sedillo
I made this tag using Photoshop CS5, but it can be worked in any program you have working knowledge of. Instructions are using a Mac, so you may need to use Ctrl where I use Command.
Here we go :)

Open your template. Image>Duplicate. Close out original. Delete credit layer. If you want to resize your template and work space, now is a good time. Image>Image/Canvas resize.

Click Raster 8 in your layer palette. Add Paper 10 above it. Right click paper 10 and choose Create Clipping Mask. Your paper should now have taken the shape of raster 8.
Click Raster 9 in layer palette. Add paper 9 using the steps above.
Raster 7- Paper 7
Raster 6 & Copy of Raster 6- Paper 6
Raster 5- Paper 1
Raster 3- Paper 5
Raster 11- Command+U, Slide Lightness scale all the way to the left, making the layer black.
Clip a close up of tube to raster 11. Blend mode Luminosity. Command+J to duplicate.
Create clipping mask on duplicate tube. Command+Shift+U to desaturate. Blend mode Screen, Opacity 85%. Clip Paper 6 to duplicate tube. Blend mode Luminosity, Opacity 50%.
Raster 4- Paper 6
Open your WA layer. Command+U. Slide Lightness scale all the way to the right, making the layer white. Add above Raster 11. Duplicate WA. Resize, change the color to Pink and add above Raster 11. 
Add your tube and drop shadow.
Now go through and decorate your tag with elements from the kit. From bottom to top I used:
frame 4
caution tape
brad 1
nail polish
zipped heart
Add drop shadows where needed.

Now let's get our mask ready! Open your mask on a new canvas.
Add a new layer above it, and flood fill with color of choice, 
or use a paper from kit .
Make sure your mask layer is activated and Command+A to Select All, Command+C to Copy.
Activate the paper layer, hit Q to turn on Quick Mask mode.
Command+V to paste the should now be tinted red.
Hit Q to turn off Quick Mask mode and there should now be marching ants on your canvas.
Hit the Add Layer Mask button at the bottom of the layer palette (rectangle with the circle in it)
Right click- convert to smart object
Right click- Rasterize layer
Drag completed mask to your tag canvas as the bottom layer
Resize it if needed.

With your Rectangular Marquee Tool (m), draw out a selection around the entire
tag, just slight smaller than the canvas. Image>Crop.
This will get rid of any stray pixels or drop shadows you don't see.
Image>Trim>Top Left Pixel Corner to remove the excess space.
Add your license and copyright info to the tag.
Add name and any other text of choice.
Merge, save as PNG and enjoy :)
